24 Health and Sanitation 605Report on Regional Public Health Services and Health Promotion Services This report is compiled annually by the Ministry of Health, Labour and Welfare in order to assess the implementation of public health policy aimed at maintaining and promoting the health of local residents according to regional characteristics by each health center and municipality, as well as to obtain basic data for the efficient and effective promotion of regional public-health policymaking by the national and local governments. The report covers all the public health centres (hokenjyo) and municipalities. The report is composed of two parts, namely, Local Health Part and Health Promotion Part. Statistics on Food Poisoning This survey is compiled annually with the aim to grasp the situation of the occurrence of victim of food poisoning and deaths by the poisoning effectively and to determine the complicated situation of occurrence. The survey covers all the health centres of the whole country. The Prefectural governments which conducted the survey enter in the investigation slip for food poising case after the survey finishes and submits to the Ministry of Health, Labour and Welfare. Patient Survey (Fundamental Statistical Survey) This survey is conducted every three years by the Ministry of Health, Labour and Welfare with the aim of clarifying the actual state of injury or disease of patients who use hospitals or clinics (hereinafter refers to as medical institution) and obtaining basic data for healthcare administration. The reference date is a day designated for each medical institution among three days of October. The patients who received treatment on the survey date at the survey institutions and those who were discharged in September are surveyed. Vital Statistics (Fundamental Statistical Survey) See "2 Population and Households". This chapter covers deaths and death rates by leading causes of death. Survey of Medical Institutions (Fundamental Statistical Survey) This survey has been conducted by the Ministry of Health, Labour and Welfare since 1953, with the aim of clarifying the distribution and conditions of hospitals and clinics (hereinafter refers to as medical institution), understanding the medical care functions of medical facilities, and obtaining basic data for healthcare administration. The survey consists of two parts: the Static Survey of Medical Institutions and the Dynamic Survey of Medical Institutions. The survey is conducted every three years with the aim to grasp the conditions of all the medical institutions, whereas the Dynamic Survey is conducted monthly on the basis of the applications and reports of opening and closing and changing submitted by medical institutions. The static survey covers all the medical institutions in Japan that are in practice at the time of survey. The reference date is as of 1 October. Statistics of Physicians, Dentists and Pharmacists This survey is conducted by the Ministry of Health, Labour and Welfare with the aim of clarifying the distribution of physicians, dentists, and pharmacists by gender, age, type of work, workplace, clinical department name, etc. and obtaining basic data for health, labor, and welfare administration. It was called the "Survey on Physicians, Dentists and Pharmacists" until 2016 and the "Statistics on Physicians, Dentists and Pharmacists" starting in 2018. The survey is conducted once every two years on the situation as of the end of December, with the respective notification forms of physicians, dentists, and pharmacists notified under the provisions of the Medical Practitioners Act (Article 6, Paragraph 3), the Dentists Act (Article 6, Paragraph 3), and the Pharmacists Act (Article 9) as the subjects of tabulation. Starting in 2018, the Survey of Physicians, Dentists, and Pharmacists was discontinued and the Statistics on Physicians, Dentists, and Pharmacists were created instead.
