  Responsible agency: Health Statistics Office, Vital and Health Statistics Division, Statistics and Information Department, Minister's Secretariat, Ministry of Health, Labour and Welfare.

Purpose: To obtain basic data on the illnesses and injuries of the patients who use hospitals and clinics for the promotion of medical and health services.

This survey had been conducted annually since 1953 as a one-day survey based on patients' clinical records. Since 1984, however, the sample and contents of the survey have been enhanced to allow representation by area, and the survey has been conduced once every three years.

Contents: The rate of estimated patients (the number of patients who used medical institutions on a survey day against a population of 100,000) was 4805 in 1960 and increased to 6987 in 1970. In 1980, it was 6847, in 1990, it was 6768, and in 2002, it was 6222. By age group, the rate of estimated patients for 65 and older patients had increased until 1990, but has declined since then. The rates for other age groups had remained almost unchanged and are now declining. The rate of outpatients for the age group of 65 and older drastically increased from 1965 to 1975. Though it declined in 1980, it had increased until 1996, and has been declining since 1999. The rates for other age groups are declining gradually.

Reference dates: Yearly between 1953 and 1983 and 1984, 1987, 1990, 1993, 1996, 1999, 2002 and 2005.

Survey frequency and date (or period): Frequency: Every three years (since 1984). Survey date: a) Hospitals: One day designated for each hospital during the three-day period from October 18 to 20, 2005. b) Clinics: One day designated for each clinic, either October 18, 19 or 21, 2005 (the case of the 2005 survey). For discharged patients, the survey period was one month from September 1 through 30, 2005) (the latest survey).

Survey method: Area covered: Nation. Unit: Establishments. (Medical institutions: hospitals, general clinics, and dental clinics. For discharged, only hospitals and general clinics are covered.) Sampling method: Stratified random sampling. Survey method : Mail survey (self-entry)

  Responsible agency: Health Statistics Office, Vital and Health Statistics Division, Statistics and Information Department, Minister's Secretariat, Ministry of Health, Labour and Welfare.

Purpose: To identify the establishment and number of medical institutions (hospitals, general clinics, and dental clinics) in Japan, regional distribution, and their specialization.

Contents: As of October 1, 2004, medical institutions numbered 174,698, with 172,685 in operation, excluding those that have been closed or closed for over one year. The number of hospitals has increased every year since 1953 when this survey started, up to 1990. It then decreased thereafter to under 10,000 in 1992 and has continued declining.  The number of general clinics with no beds are increasing yearly.

Reference dates:1948 through 1972 (yearly by the end of December). Monthly since November 1973 (monthly survey: Dynamic Survey of Medical Institutions). From 1975, Static Survey of Medical Institutions was added and is conducted every three years.  (For 1975, 1978, and 1981, the survey was conducted as of the end of December, while in 1984, 1987, 1990, 1993, 1996, 1999, 2002 and 2005, it was conducted as of October 1.)

Survey frequency and date (or period): Frequency: Dynamic survey - Monthly (since 1975); Static survey - Every three years (since 1975). Survey date: Dynamic survey - Every month; Static survey- October 1, 2005 (the latest survey)

Survey method: Area covered: Nation. Unit: Establishments. (Medical institutions: hospitals, general clinics, and dental clinics. Dynamic surveys cover only hospitals, general clinics, and dental clinics which file reports of establishment, closure, or modification or are disposed of under the Medical Service Law.) Sampling method: Complete enumeration. Survey method: Mail survey (self-entry). For dynamic surveys, statistics were developed based on administrative records. (After receiving or disposing reports subject to the Medical Service Law, the relevant prefectural governor prepares survey forms for submission to the Minister of Health, Labour and Welfare.)

Tabulation items: Numbers of hospitals by area, founder, type and capacity; number of general hospitals by number of beds, founder and clinical department; numbers of general clinics by area, division between facilities with beds and those without beds, and founder; number of beds; number of dental clinics and beds at dental clinics; number of hospitals by establishment, closure, area, and founder; number of general clinics and dental clinics.

Name and date of publication: "Reports on Survey of Medical Institutions" is published every March two years after the survey. In years with no static survey, a corrected report of the previous static survey, which takes into account the results of the dynamic survey, is published.

  Responsible agency: Health Statistics Office, Vital and Health Statistics Division, Statistics and Information Department, Minister's Secretariat, Ministry of Health, Labour and Welfare.

Purpose: By covering patients who have used medical institutions (hospitals), to survey the state of receiving medical treatment, their degree of satisfaction about the medical treatment that they received, thereby to survey patients' opinions about medical services and behavior of receiving such services, in order to obtain basic data for the future medical service administration.

Contents (2002 survey): On the average, about half the outpatients waited for "less than 30 minutes" until they were called in for consultation, and the consultation lasted for "less than 10 minutes" according to about 70% of the outpatients. The ratios of patients who were provided with medical explanation were 80% among outpatients and 90% among inpatients, of whom 90% (both outpatients and inpatients) "understood" the explanation. For both outpatients and inpatients, less than 10% "saw" their clinical file by so requesting. As for the general degree of satisfaction about the hospitals that they received medical treatments at, about half of both outpatients and inpatients were "satisfied" while less than 10% were "not satisfied".

  Responsible agency: Analytical Research Planning Division, Lifelong Learning Policy Bureau, Ministry of Education, Culture, Sports, Science and Technology.

Purpose: To obtain basic data on physical development and health of school children and infants for school health administration.

Contents:
(1) Based on the results of the FY 2005 survey, the changes in weight, height, and sitting height (all national averages)are summarized as follows:
i The weight of boys aged 11, 14, 15 and 17 increased from the previous year, and the weight of those aged 17 marked the record high. For other ages, the weight declined for the boys aged 8 and 9. The weight of girls aged 8, 14 and 17 increased from the previous year, with the last one marking the record high. In contrast, the weight of girls aged 9 to 12, 15 and 16 decreased.
ii The height of boys aged 8 to 10 and 14 increased. The height of boys is decreasing in age groups from 5 to 7 and 12 from the previous year, while the height of girls is increasing in age groups of 7, 14, 16 and 17 from the previous year. In other age groups of girls, 5, 10 and 12 are decreasing from the previous year.
iii As for sitting height, a comparison with the figures for 1975 which are those for the generation of the parents of the children surveyed, the children showing the largest differences are as follows: boys aged 13, with sitting height increasing by 1.9 cm in comparison with their parents' generation's figures, and girls aged 11, with the increase being by 1.4 cm. In contrast, the sitting height of both boys and girls aged 5 decreased by 0.1 cm.
   
(2) Rates for illnesses and disorders reveal the following:
i The rate for decayed teeth is 54.4% in kindergarten, 68.2% in elementary school, 62.7% in junior high school, and 72.8% in senior high school, and decreased at all levels of the school system from the previous year. The rate for decayed teeth was on an upward swing in 1975, marking over 90% at all levels of the school system, but has been decreasing in recent years. The rate for decayed teeth by age increased in all age groups except 5 and 12, exceeding 60%. The rate for the age group of 17 was 77.0%, marking the highest level.
ii The ratio of those with unaided eyesight under 1.0 (20/20 in English) recorded 20.4% in kindergarten, 26.5% in elementary school, 47.8% at junior high schools, and 58.4% in senior high school, declining in kindergartens and senior high schools.

Please note that this is a sample survey and therefore subject to sampling errors in interpretation of the results.

Survey method: Area covered: Nation. Unit: Schools (elementary schools, junior high schools, senior high schools, secondary education schools, and kindergartens). Sampling method: Random sampling. Survey method: Survey by mail (self-entry).

Tabulation items: Correlation between height and weight/sitting height and averages, contraction of illnesses, and weight trends (obesity and underweight).

Name and date of publication: The results are published at the end of December of the survey year as "Preliminary Report of School Health Survey", followed by publication of "Report on School Health Survey" in March of the year following the survey.

Additionally, to prepare a list of instittions and establishments for long-term care.

Purpose: Based on the Health Promotion Law, to survey the people's physical conditions, intake of nutrients, etc. and state of life-style, so as to obtain basic data for the comprehensive promotion of their health.

Contents: According to the National Health and Nutrition Survey in 2003, the daily energy intake per Japanese was 1,920 kcal, just about an appropriate average amount.  The ratio of fat energy, however, exceeded appropriate levels, necessitating consideration of preventive measure for diseases attributable to lifestyle.

The results of the 2003 survey show that over 30% of men in the age groups from 30 to 69 were obese people with BMI's (Body Mass Index, body weight (kg) / height (m)2) 25 and over, increasing compared to 20 years ago in any age group. Moreover, about 30% of men aged 30 to 69 were suspected of upper-body obesity (BMI's 25 and over and abdominal circumferences of 85cm and over).

Survey frequency and date (or period): Frequency: Yearly. Survey date: A selected day in November

Survey method: Area covered: Nation. Unit: Households and individuals. Sampling method: Random sampling. Survey method: Enumerator survey (enumerator's entry). The enumerators are doctors, registered dieticians, public health nurses (PHN's), etc. Principally, registered dieticians are in charge of the surveys on nutritional intake. Doctors, PHN's, registered nurses (RN's), medical (clinical) testing technicians and their assistants are in charge of the surveys on the physical body.

Tabulation items:

-    Items related to physical condition: Height, weight, abdominal circumference, blood pressure, blood test results (for total cholesterol, blood-sugar level, etc.), amount of exercises per day (walk count), use of medicines, exercising habits, etc.
-    Items related to nutritional intake: Nutrients, etc. intake of foodstuffs by type of food, state of eating such as not eating and eating out, etc.
-     Items related to life-style in general: Eating habits, physical activities and exercises, resting (sleeping), drinking, smoking, condition of teeth, etc.

Purpose: To obtain basic data on administration of sanitation in prefectures, designated cities, and core cities for managing national and local sanitation.

[Outline of Survey]

Reference dates: This survey can be traced back to the 1886 "Report to the Ministry of Home Affairs" (under an ordinance of the prewar Ministry of Home Affairs ("Naimusho", 1873 - 1947)). The current system was adopted in 1954. The previous and current reports are commonly featured since they both consolidate items not covered by any other reporting systems, although there have been a number of modifications due to enactment and abolition of laws and administrative necessity.

Survey frequency and date (or period): Frequency: Every fiscal year or every other fiscal year (since 1997), depending on items subject to the survey. Survey date: Annual report: As of March 31. Biennial report: As of December 31.

Survey method: Area covered: Nation. Unit: Establishments. Sampling method: Complete enumeration. Survey method: Survey by mail (self-entry): Prefectural governors, mayors of the designated cities, or mayors of core cities complete specified report forms for the subject period or enter current data to provide information on specified items for submission by the due date to the Managing Director of the Statistics and Information Department of the Minister's Secretariat, Ministry of Health, Labour and Welfare.

Tabulation items: Mental health (hospitalization by order of the governor, applications for public support); nutrition (issuance of nutritionist's certificates, school-lunch facilities); environmental sanitation (cemeteries, cremation, barbers' and beauty salons, laundry services); food sanitation (food services, food sanitation administrators); dairy and meat sanitation (test on the collection and disposal of dairy products, amount of processed dairy products); medical monitoring; pharmaceutical (approval for operation of pharmacies, pharmacies, poisons, and powerful chemicals), protection of mother's body (sterilization operation, abortion), etc. A table of operating medical workers (public health nurses (PHN's), licensed midwives, licensed nurses, assistant nurses, dental hygienists, dental technicians, massage, acupuncture, moxibustion, and judo therapists) is compiled every other year (in even-number years).

  Responsible agency: Social Statistics Division, Statistics and Information Department, Minister’s Secretariat, Ministry of Health, Labour and Welfare.

Purpose: To gain a quantitative understanding of the state of welfare administration in every prefecture, designated cities or core cities as various regulations pertaining to social welfare are executed, and to obtain basic data for the conduct of social welfare administration by the national and local governments.

[Outline of Survey]

Year of survey implementation: There is the report entitled "Report to the Ministry of Home Affairs" (under an ordinance of the prewar Ministry of Home Affairs ("Naimusho", 1873 - 1947)) going back to 1886.  In 1938, the report became Reports to the Health and Welfare Ministry.  Since then, the reports have been revised on numerous occasions for administrative reasons including the establishment, revision, and abolishment of laws and regulations.  Now, the report is being made for report on social welfare administration and services for fiscal 2000 and subsequent years.

Date and period of the survey: Period - Monthly and fiscal year reports depending upon the items surveyed
Dates of implementation: Monthly reports on the last day of each month.  The fiscal year report comprehensively covers the situation for the year concerned.
